Function & Flow Therapy is committed to protecting your privacy and handling personal information respectfully, securely, and in accordance with Australian privacy legislation, including the Privacy Act 1988, Australian Privacy Principles, and relevant NSW health records legislation.

What information we collect

We collect personal and health information necessary to provide occupational therapy services. This may include:

  • Contact details and identifying information

  • Health and medical information relevant to therapy

  • Information about participation, environments, and supports

  • Medicare, NDIS, or other funding details for claiming purposes

We only collect information that is relevant and required for service provision.

How we collect information

Information is collected directly from you where possible, and may also be provided by authorised third parties such as parents/guardians, doctors, schools, support coordinators, or funding bodies. Consent is obtained prior to assessment and service delivery.

How we use and share information

Your information is used to provide safe, effective, and coordinated occupational therapy services.
We may share relevant and necessary information only with:

  • Other health or education professionals involved in your care

  • Equipment suppliers or service providers (limited to required details only)

  • Funding bodies such as the NDIS

Information is not shared without consent unless required or authorised by law, including mandatory reporting obligations or where there is a serious risk to health or safety.

Storage and security

All records are stored securely in electronic, cloud-based practice management software hosted in Australia (Splose). Reasonable steps are taken to protect information from loss, misuse, or unauthorised access. Hard copy records are securely destroyed once digitised.

Use of AI and digital tools

Function & Flow Therapy uses Splose, which may include AI-supported features to assist with administrative tasks such as drafting clinical notes, reports, or summaries. AI tools are used to support efficiency and accuracy only and do not replace clinical judgement. All AI-generated content is reviewed, edited, and approved by a qualified occupational therapist before being finalised or shared. No information is used for AI purposes beyond what is permitted within the secure Splose system.

Access and correction

You (or your authorised representative) may request access to, or correction of, your personal information. Requests must be made in writing and will be responded to within a reasonable timeframe, subject to legal exceptions.

Privacy concerns

If you have concerns about how your information is handled, please contact Function & Flow Therapy in writing. We take privacy concerns seriously and aim to resolve them promptly. You may also contact the Office of the Australian Information Commissioner if required.

Changes

This privacy summary may be updated or modified periodically to reflect changes in legal requirements or practice operations.
